5/8/2023 EnchantaVaughn Approved (AM)When the school day began, client is reported running and jumping around the classroom and being disruptive. Client struggles with staying still and displays excessive movement throughout the morning. Client presents frustration when directed to complete simple assignments and to pay attention in class. Teacher reports client have been having a difficult time staying on track and making good choices when he is distracted by others who are horseplaying and causing interruptions. Client is given several chances to get his act together. Client did a better job by going to lunch in a more polite way and listening to his teacher when directed to get in line and walk with his class. (PM) Client was respectful and polite to staff while showing good manners. Client picked up his food and sat down and seemed to be happy and content with being around his friends. Client displayed good social behaviors in the cafeteria. Client was respectful and polite to cafeteria staff. Client displayed normal activity before transitioning to see his counselor and begin group session. Client demonstrated excellent interaction by communicating his feelings and sharing his experience when asked about his day. Client exhibited a positive attitude and did a great job reflecting and engaging in the listening activity. Client asked appropriate questions and gave appropriate feedback. (EOD)Client was able to distress and calm down as the session continued. Client continued to avoid inappropriate behaviors throughout session and was praised for being on task. Client was educated about school rules and regulations and why it’s important to follow rules. Client showed understanding and commitment with working on his objectives. When client returned to class following psycho social break he went to finish his assignments. Teacher applauded client on his consistent positive behaviors and was happy to see him do well on his own. Client have been doing a much better job calming down and refocusing back on his assigned work with minimal TDT support.
